A painful facial blemish during treatment deserves an assessment if it persists.

Try a warm compress

AAD suggests a warm, damp washcloth for 10 to 15 minutes, three times daily, for a deep, painful pimple. Use a clean cloth each time and resist popping it. [1]

Know when home care is insufficient

AAD recommends a dermatologist if the deep pimple persists despite its home-care advice or if several deep, painful pimples develop. These can require prescription medicine or procedures. [1]

Ask which procedure fits the blemish

AAD describes extraction for blackheads and whiteheads, usually after other treatment fails. A clinician may instead consider a corticosteroid injection for one especially painful acne nodule or cyst. It is not a treatment for multiple blemishes; excessive corticosteroid can cause unwanted effects. Only a doctor should inject it. [2]
