Before starting a prescription to restore pigment, agree on what improvement would matter to you and when you will review it. For Opzelura, a facial study score, visible color change and a decision to continue treatment need separate discussion.
Confirm the diagnosis before choosing the cream
Opzelura is ruxolitinib cream. Its U.S. vitiligo indication covers nonsegmental vitiligo in people aged 12 and older. [1] That does not make it a general cream for pale patches noticed during GLP-1 treatment.
The American Academy of Dermatology (AAD) explains that color loss can also follow a skin injury or reflect another condition such as tinea versicolor. Diagnosis involves a history and examination, sometimes with a Wood's lamp. Treatment is a choice to discuss after assessment. [2]
Our pale-patch assessment guide covers that first appointment. Tell the clinician when the change began and bring the names of your medicines and skin products.
Read the facial endpoint correctly
The original report describes two randomized trials enrolling 674 people aged 12 or older, with total vitiligo involvement up to 10% of body surface area. The primary endpoint was at least 75% improvement in the facial Vitiligo Area Scoring Index, or F-VASI75, at week 24. [4]
| Label results at week 24 | Opzelura | Vehicle cream |
|---|---|---|
| TRuE-V1: F-VASI75 | 29.9% | 7.5% |
| TRuE-V2: F-VASI75 | 29.9% | 12.9% |
These are the figures in the current U.S. label's Table 6. [1] The endpoint does not mean 75% of participants recovered all their pigment. Nor does a facial endpoint establish the same response on fingertips or every other body area.
Ask which area your own review will assess and how improvement will be documented. If facial appearance is your priority, say so. If another site matters more, ask for evidence and expectations specific to that site.
Put the review date in the plan
The U.S. label says satisfactory response may take more than 24 weeks; if repigmentation is not meaningful to the patient by then, the clinician should re-evaluate. [1] After week 24, both trial groups could receive active cream. The later period therefore did not maintain the same vehicle comparison. [4]
More time on treatment is not a guarantee of the result you want. Ask what would support continuing, changing the plan or discussing another option. Record the answer alongside the follow-up date and contact instructions for problems before that visit.
AAD notes that treatment does not cure vitiligo and that returned pigment may fade. Maintenance can become part of the discussion. [2] Ask about that possibility before judging the entire commitment from the first prescription.
Review the prescription as a medicine
Opzelura carries boxed warnings about serious infections, mortality, malignancy, cardiovascular events and thrombosis; some evidence comes from oral JAK inhibitors in other populations. Ask the prescriber to review the complete label and your medical history. [1]
Bring the full medicine list, including the exact GLP-1 product, rather than assuming that a skin cream needs no coordination with other care. This article does not establish a GLP-1 interaction, a different response during weight loss or a reason to change either prescription yourself.
Keep protection separate from pigment goals
AAD advises shade, protective clothing and broad-spectrum, water-resistant SPF 30 or higher outdoors. Reapply sunscreen every two hours and after swimming or sweating. Skin that has lost pigment sunburns easily; tanning can also increase contrast. [3]
Discuss optional camouflage if you want it. Choosing coverage, choosing treatment and choosing to leave the patches visible are personal decisions. Ask how any cosmetic product fits with the instructions for your prescription.
Checked October 28, 2026. Trial percentages above come from the U.S. prescribing information revised June 2026, not a mixture of label and journal analysis figures. The original report states that its sponsor funded medical writing assistance. AAD's vitiligo pages acknowledge support from Incyte Dermatology and state that AAD solely developed their content. This is education for a prescribing discussion, not a treatment recommendation.
