Before using a tirzepatide headline to judge your own muscle loss, check the denominator. A percentage of starting lean mass and a percentage of weight lost answer different questions. The SURMOUNT-1 body-composition report contains both.
Who contributed the scans?
The analysis included 160 participants: 124 receiving tirzepatide, with 5, 10 and 15 mg doses pooled, and 36 receiving placebo. Although 255 enrolled in the substudy, the analysis required baseline and follow-up DXA scans. This is a smaller sample within SURMOUNT-1, not a body scan of every trial participant. [1]
Work through the denominators
Consider an invented example, separate from the study. Someone starts with 50 kg of lean tissue and later has 46 kg. That is a 4 kg decrease: 4 ÷ 50 × 100 = 8% of starting lean tissue.
Suppose the same person also loses 12 kg of fat. Among those two tissue changes, 4 kg out of 16 kg is lean tissue: 4 ÷ 16 × 100 = 25%. The 8% and 25% describe the same example without contradicting each other.
Those invented figures are an arithmetic illustration, not an expected outcome or treatment target. When reading a report, write the denominator beside each percentage before comparing it with another result.
Lean tissue is not a muscle-only measurement
DXA separates fat, bone mineral and lean soft tissue. The paper's lean-mass measure excludes bone mineral; it does not isolate skeletal muscle or measure strength. Several authors were Eli Lilly employees and shareholders. [1]
A scan result alone cannot answer whether you can rise from a chair, carry shopping or climb your usual stairs as well as before. Bring a specific change in those activities to the appointment. Ask which assessment would help explain it rather than translating the DXA percentage into a percentage of lost strength.
Check the correction when reading Figure 4
An August 2025 correction changed the fat- and lean-mass graph axis labels in Figure 4 from percent to kilograms. If you saved an earlier image of that figure, consult the corrected version before quoting its values. [2]
Make the follow-up plan specific
A 2025 joint advisory from four nutrition and obesity organizations recommends nutritional assessment and adequate protein alongside strength training. It cautions that increasing protein alone is likely insufficient to preserve muscle without structured resistance exercise. [3]
Ask for a plan you can describe on paper: which foods fit your appetite, how the protein target was chosen, which exercises suit your abilities and when progress will be reviewed. If smaller meals or persistent symptoms are interfering with eating, include that in the discussion. A supplement purchase does not supply the missing exercise instruction or nutrition assessment.
For related evidence, see the exercise and liraglutide trial. For a broader appointment checklist, read body shape, food and strength planning.
